CLTS.
START WITH THE CHILD.
CLTS is a Wisconsin Medicaid home and community-based waiver program for eligible children with disabilities. It funds approved supports that help children grow, build independence and remain connected to family and community life.
Request a CLTS ConversationELIGIBILITY IS DECIDED BY THE COUNTY.
Wisconsin DHS states that a child must be younger than 22, eligible for Wisconsin Medicaid, live at home or in another approved setting, meet an eligible level of care and be able to receive required care safely at home or in the community. The county waiver agency completes the application and functional eligibility process.
UNDER 22
The child must be younger than age 22.
MEDICAID ELIGIBLE
The child must meet Wisconsin Medicaid eligibility requirements.
HOME + COMMUNITY
The child must be able to receive safe, required care in an approved home or community setting.
Achieving Individuals does not determine CLTS eligibility. Families apply through their county health and human services agency.
AUTHORIZED GOALS.
PRACTICAL SUPPORT.
Depending on the child’s assessed needs, individual service plan, provider availability and county authorization, Achieving Individuals can discuss support in areas such as:
RESPITE
Planned relief for unpaid caregivers while the child receives safe, individualized support.
DAILY LIVING SKILLS
Practice with routines, organization, communication, problem-solving, community navigation and age-appropriate independence.
MENTORING
Goal-focused relationship support that strengthens confidence, self-direction, accountability and participation.
COMMUNITY PARTICIPATION
Support practicing skills in everyday settings and increasing meaningful participation in community life.
FAMILY COLLABORATION
Clear communication with caregivers about goals, routines, progress and what helps support consistency.
COORDINATED SERVICES
Collaboration with the support and service coordinator and other approved team members when authorized and appropriate.
FROM INQUIRY TO AUTHORIZED SUPPORT.
Start with the county
If the child is not enrolled, contact the county health and human services agency and ask about CLTS. The county guides the application, home visit and eligibility process.
Build the plan together
If eligible, the family works with a support and service coordinator using Wisconsin’s Deciding Together approach to identify the child’s goals, needs and appropriate supports.
Discuss provider fit
The family or coordinator may contact Achieving Individuals to discuss goals, service type, availability, location, schedule and whether the requested support is a fit.
Obtain authorization
The county must authorize the specific CLTS service in the child’s plan before CLTS-funded services begin.
Deliver and coordinate
Services follow the authorized plan, with appropriate communication and documentation to support continuity and progress.
HELP US UNDERSTAND THE FIT.
CHILD + GOALS
Share the child’s age, the support being requested and the practical goal the team wants to address. Do not send diagnoses or sensitive medical details through the website.
CLTS STATUS
Tell us whether the child is enrolled, applying or only exploring the program.
COORDINATOR + COUNTY
If enrolled, include the support and service coordinator’s name and county waiver agency so next steps can be coordinated.
