Terms & Conditions

Last updated September 6, 2026

These Terms & Conditions govern your use of the Achieving Individuals website and its service-request forms. By using this website, you agree to these terms.

Website information

The information on this website is provided for general informational purposes. It describes potential services and does not guarantee eligibility, availability, acceptance, scheduling, funding or a particular result.

No clinical, legal or emergency services

Achieving Individuals provides nonclinical youth-development, practical skill-building, respite, mentoring, writing and community-based support. The website and services are not therapy, medical care, diagnosis, legal advice or emergency intervention.

If someone is in immediate danger, call 911. For suicide or mental-health crisis support in the United States, call or text 988.

Service requests

Submitting a website form begins a conversation only. It does not create a provider-client relationship, confirm an appointment or obligate Achieving Individuals to provide services. Services begin only after fit, availability, requirements, fees, consent and any necessary written authorization are confirmed.

You agree to provide accurate contact and scheduling information and to submit information only when you are authorized to do so. Do not submit Social Security numbers, financial account information, medical records, diagnoses or other highly sensitive information through the website.

CLTS-funded services

Achieving Individuals does not determine eligibility for Wisconsin’s Children’s Long-Term Support Program. CLTS-funded services may begin only when the appropriate county waiver agency authorizes the service and includes it in the child’s approved individual service plan. A website request is not county authorization.

Young people and caregiver authorization

A parent, legal guardian or other properly authorized adult must provide any consent required for services involving a minor. A person submitting information about a young person represents that they are authorized to share it.

Fees, scheduling and cancellations

Any applicable fees, funding arrangements, schedules, locations, cancellation expectations and service-specific terms will be confirmed before services begin. Website descriptions do not override a written service agreement, county authorization or approved service plan.

Intellectual property

Website text, branding, program names and original materials belong to Achieving Individuals or their respective owners and may not be copied, republished or used commercially without permission, except as allowed by law.

Third-party resources

Links to government agencies and other third-party resources are provided for convenience. Achieving Individuals does not control their content, availability or privacy practices.

Availability and limitation

The website may be changed, interrupted or unavailable. To the extent permitted by law, Achieving Individuals is not responsible for losses caused solely by reliance on general website information, third-party resources or temporary website unavailability. Nothing in these terms limits rights or responsibilities that cannot legally be limited.

Privacy

Information submitted through the website is handled as described in the Privacy Notice.

Changes and governing law

These terms may be updated by posting a revised version on this page. Wisconsin law governs these terms, except where another law must apply.
